We presented here a case of hanging, in which death occurred after 28 days of incident. A female had committed suicidal attempt by hanging herself at home and saved at that time, then referred to hospital and treated accordingly, but at the end of 28 days, female died due to irreversible brain damage and respiratory arrest. We reported this case for its rarity, for the discussion of the possible delayed causes of death in case of hanging, to explain the circumstances of such a possibility and to emphasize the importance of forensic medicine experts to give final opinion regarding the cause of death by post mortem examination.
