Frictional trauma to keratinocytes secondary to scratching may result in apoptosis-like degenerative changes of epidermal keratinocytes and give rise to keratinocyte-derived amyloid deposits in the epidermis. Ultraviolet light-induced keratinocyte damage was thought to be one of the etiologic factors responsible for so called __ampersandsignlsquo;actinic macular amyloidosis. We studied clinical and histologic profile of untreated macular amyloidosis limited to photoexposed area. Methods: A total number of 37 patients with macular amyloidosis in photoexposed area, attending Dermatology out-patient of a tertiary care hospital in central India over a period of 2 years, were included in the study. Clinical and histologic profile of these patients was studied. A detailed history and clinical examination was done followed by histopathology in consenting patients. Results: Among the 37 patients, 27(72%) were females and 10 (28%) were males. Most of the patients (37%) were in the age group of 17-27 years of age. Sparing of hair follicles in the affected skin was seen in 20 (54.05%) patients. In most of the patients upper back and extensor aspect of upper extremities was involved. A total of 20 (54%) patients were asymptomatic while 17 (46%) patients had mild pruritus. None of our patients complained of photosensitivity. On enquiry, a total of 21 patients gave history of chronic sun exposure (21/37, 56%). On histopathology, Orthokeratosis of Basket weave type was observed in majority of cases (66.66%) along with normal granular layer (100%). Epidermis was normal in majority of cases (80%). Amyloid deposits were not noted in majority of cases (93.33%). Melanophages were noted in 53.33% of patients, however, interface changes were noted in only 1 patient. Conclusion: Our study suggests that macular amyloidosis in photoexposed area is a variant of macular amyloidosis with lesser known etiologic factors and unremarkable histologic features. Role of ultraviolet light in the causation of this entity needs to be explored by further studies.&#13;
