The immediate spica cast is safe and effective for children up to 6 years of age1. Compression plate provides rigid anatomic fixation but requires extensive dissection. Fixation of the fractures with flexible intramedullary nailing is a safe and effective alternative treatment method, especially in simple transverse and short oblique fractures1. The present study is done to compare the clinicoradiological outcomes of closed post traumatic femoral shaft fracture in 6-12 years age of children stabilised either by titanium elastic nailing or dynamic compression plating. Materials and methods: The study is a prospective randomised control trial of 40 cases(24male and 16 female) of closed femoral shaft fractures of age 6-12 years who underwent titanium elastic intramedullary nailing or open reduction and dynamic compression plating (20 in each group),conducted for a period of 22 months with a minimum follow up period of 16 months. Results: Average time of union in Titanium elastic nailing group was 11.1 weeks and in Dynamic compression plating group it was 11.3 weeks which was statistically insignificant (p-value 0.338) and so was the finding on comparing the functional outcome. Mean operating duration and average operative blood loss between the two groups was found to be statistically significant. Conclusion: There is no considerable difference between titanium elastic nailing or open reduction and dynamic compression plating in terms of final functional outcome however post-op complications were found to be less when operated by elastic nailing.&#13;
