Case report: A 37 year male patient sustained a bilateral medial malleolar fracture following a road traffic accident with an undisplaced posterior malleolus fracture on the right side. The patient underwent osteosynthesis of both the ankles in the same sitting with malleolar screws with no significant complaints at 3 year follow up. Discussion: In the present case, the right side had a vertical medial malleolar fracture representing a supination-external rotation type injury while the left side had transverse fracture represented a pure abduction injury. Conclusion: The case has been reported in view of its rarity alone with its probable mechanism of injury. Surgical management resulted in a good clinical outcome for the present case.&#13;
