Borkar, Professor and Head, Department of Pharmacology, NKP Salve Institute of Medical Sciences, Hingna Road, Digdoh Hills, Nagpur, (MS), India</Affiliation><Abstract>Objectives: To compare the efficacy and safety of four atypical antipsychotic drugs with each other (aripiprazole, risperidone, olanzapine, quetiapine) in the treatment of delirium, with patient&#x2019;s age taken into account. Methods: This study was a 7 day prospective, comparative clinical study of efficacy and safety among four atypical antipsychotics in the treatment of delirium at a tertiary care hospital in central India. The patients with symptoms, suggestive of delirium were confirmed for diagnosis and treatment was started, after enrolling them in this study by the department of Psychiatry. A total of 70 subjects were allotted to receive either aripiprazole (n=20), risperidone (n=19), olanzapine (n=16) or quetiapine (n=15). The efficacy was evaluated by Delirium Rating Scale (DRS) - Revised 98 and safety by Udvalg Kliniske Undersogelser (UKU) side effect rating scale. Results: At baseline, the differences in mean DRS scores and the rates of improvement among the four groups were not statistically significant. The treatment response was comparatively lower in patients older than 70 years, than patients below 70 years age and this effect was similar in all four study groups. Seventeen patients had few adverse effects, but no significant differences in terms of adverse effects were found between four groups. Conclusion: Aripiprazole, risperidone, olanzapine and quetiapine showed same efficacy and safety profile in treating delirium patients. But, from our findings, it can be inferred that age factor needs to be considered critically, while administering any antipsychotic drug.&#xD;

Islam</Author><AuthorLanguage>English</AuthorLanguage></AuthorList><Abstract>Aim: The Aim of this case report is to describe a rare case of bilateral medial malleolar fracture with its probable mechanism of injury. Case report: A 37 year male patient sustained a bilateral medial malleolar fracture following a road traffic accident with an undisplaced posterior malleolus fracture on the right side. The patient underwent osteosynthesis of both the ankles in the same sitting with malleolar screws with no significant complaints at 3 year follow up. Discussion: In the present case, the right side had a vertical medial malleolar fracture representing a supination-external rotation type injury while the left side had transverse fracture represented a pure abduction injury. Conclusion: The case has been reported in view of its rarity alone with its probable mechanism of injury. Surgical management resulted in a good clinical outcome for the present case.&#xD;

Materials and Methods: The present prospective study was conducted at a tertiary care hospital and post graduate teaching institute. Total 149 patients of acute uncomplicated cystitis who had significantly positive bacterial culture for single bacterium species were selected for the study. Eight antibiotics were tested by modified Kirby Bauer disc diffusion method. Results: The uropathogens identified were Escherichia coli (78.53%), Klebsiella pneumoniae (4.70%), Proteus mirabilis (2.01%), Staphylococcus saprophyticus (12.75%) and Enterococcus fecalis (2.01%). Most isolates showed resistance against cotrimoxazole (trimethoprim plus sulfamethoxazole) and coamoxiclav (amoxicillin plus clavunalate). They were susceptible to fluoroquinolones, third generation cephalosporin (cefpodoxime) and nitrofurantoin. Conclusions: The most effective antibiotics in the present study for acute uncomplicated cystitis were levofloxacin and cefpodioxide&#xD;
